Radial shockwave is the more widely available type and is highly effective for conditions located in superficial tissue — within approximately 0–3 cm of the skin surface. It excels for:

  • Plantar fasciitis (the plantar fascia is accessible from the heel surface)
  • Lateral epicondylitis / tennis elbow (the common extensor tendon is superficial)
  • Myofascial trigger points and muscle tension
  • Patellar tendinopathy in lean patients
  • Greater trochanteric pain syndrome (hip, for patients with lower BMI)

Radial shockwave devices are generally less expensive, which is why they’re more widely available. When properly applied to the right conditions, radial shockwave produces excellent clinical outcomes — the majority of published research on plantar fasciitis and tennis elbow uses radial devices.

When Focused Shockwave Is Used

Focused shockwave reaches conditions located deeper in the body, where radial energy doesn’t penetrate effectively. It’s typically preferred for:

  • Calcific shoulder tendinopathy (the supraspinatus tendon lies 3–5 cm beneath the skin)
  • Deep hip pathology (psoas, labral-adjacent tendons)
  • Achilles tendinopathy in patients with greater tissue depth
  • Non-union fractures and bone healing applications

Does the Type of Device Matter to Your Outcome?

Yes — when matched correctly to your condition. The clinical evidence for both types is strong when the device is appropriate for the pathology being treated. A radial device will underperform for deep shoulder calcification. A focused device applied to a superficial plantar fasciitis may over-penetrate and miss the target tissue.
