Here’s what you actually need to know about the side effects of shockwave therapy.

Expected Effects vs. Side Effects

There’s an important distinction between expected treatment effects and side effects. When you take ibuprofen for pain, stomach irritation is a side effect — an unintended consequence. But when shockwave therapy causes temporary soreness in the treated area, that is actually part of the mechanism of action, not a side effect. Understanding this distinction matters.

What Most Patients Experience During Treatment

During a shockwave session, the applicator is pressed against the skin over the treatment area and delivers rhythmic acoustic pulses. Patients typically describe this as:

  • A tapping or pulsing sensation, sometimes described as “like a tiny jackhammer”
  • Discomfort that ranges from mild to moderate, especially directly over inflamed tissue
  • Occasional sharp zingers if the applicator passes over a nerve or highly sensitized spot
  • A sensation that becomes less intense as the session progresses (many patients note that the first 30 seconds is the most uncomfortable part)

What Happens in the 48 Hours After Treatment

The post-treatment window is where patients most commonly notice effects:

  • Soreness at the treatment site (most common): About 70–80% of patients experience some degree of soreness, aching, or tenderness in the treated area for 24–48 hours. This is the inflammatory response being triggered. It typically peaks around 12–24 hours post-treatment and resolves on its own.
  • Redness or flushing: The skin over the treatment area may appear pink or red immediately after treatment, similar to mild irritation. This typically resolves within a few hours.
  • Bruising: Minor bruising at the treatment site occurs in roughly 10–15% of patients. It’s more common in patients taking aspirin or other blood thinners and in areas with little subcutaneous tissue (like the heel). Typically resolves within 3–5 days.
  • Temporary increase in pain: A subset of patients (approximately 15–20%) experience a temporary flare of their baseline pain in the first 24–72 hours after treatment. This almost always resolves and is followed by progressive improvement. It does not mean the treatment is failing.
  • Swelling: Mild localized swelling is possible, particularly over tendon-rich areas. Rare and self-resolving.

What Doesn’t Happen With Shockwave

Unlike many medical treatments, shockwave therapy does not cause:

  • Systemic side effects (no nausea, dizziness, or fatigue)
  • Organ effects (no liver or kidney impact)
  • Dependency or tolerance
  • Hormonal disruption (unlike cortisone)
  • Immune suppression
  • Changes to blood values or lab work

Because there’s no anesthesia, injection, or incision, the systemic risk profile of shockwave therapy is essentially zero. This makes it particularly attractive for patients who are sensitive to medications, have comorbidities that make injections risky, or simply want to avoid adding more chemical interventions to an already complicated medical picture.

Rare Events

In a very small number of patients, the following have been reported:

  • Hematoma (deeper bruising under the skin) — very rare, more common with anticoagulant use
  • Skin petechiae (tiny burst capillaries appearing as small red dots) — cosmetic only, resolves within days
  • Aggravation of inflammation requiring a longer treatment gap — managed by adjusting protocol spacing

Serious adverse events such as nerve injury or tendon rupture are extraordinarily rare and almost always associated with treatment in contraindicated areas or with inappropriate equipment settings. Under qualified supervision, these risks are negligible.

Managing the Post-Treatment Period

General guidelines include avoiding anti-inflammatory medications and ice for 48 hours (to protect the healing response), reducing high-impact activity for 24–48 hours, and staying hydrated. Acetaminophen is acceptable if pain management is needed during this window.
